TBH, after buying those practice exams, I don't find the NBME explanation super helpful. Most of the time, they give you a 1-liner reason for why the answer is right and then repeat that 1-liner in the "concept to learn" portion at the end of the explanations. If you are expecting UWorld level explanations, the NBMEs won't provide that.

All shelf tests are essentially medicine tests. I had quite a few questions on fluids, causes of tachycardia, causes of thrombocytopenia, etc. You probably already know this, but avoid picking the surgery-related answer just because it's a surgery shelf. Pestana and Uworld are good and will get you pretty far.
